I thought only English people who knew about dentistry were upper class English people like my relatives.

Well, this whole thing apparently started when they took her to have a baby tooth extracted, so it doesn't sound like anyone involved was terribly knowledgeable.

It was probably rotted out.  My youngest had to have a baby tooth extracted.  When he was about 4 he discovered how to pilfer from a large jar of hard candy and was chewing on them in his bed at night.  We pulled his bed back and there were wrappers everywhere.  After we explained to him that sugar rots teeth, especially when left on them all night, he became obsessed with his dental hygiene.  I am proud to say that he had a mouth full of cavlity-free adult teeth.  Of course, we have good dentists here...not quacks who will extract a whole mouth full of teeth from a child.
