None of that surprises me, but my experiences have - for the most part - been pretty positive. Few restaurateurs want to alienate loyal customers, and the restaurant where I cook (but not for which I work - long story) is very good about tossing leftovers and using good ingredients. When I'm working with my own cooks, my rule of thumb is "if you wouldn't eat it yourself, don't serve it".
However, when I was working in catering a few years back, I did see a lot of weird shit. There was the time we were serving more than 700 people at a San Francisco Fashion Week event, and we discovered far too late that not enough plates had been packed to cover the dessert course. We ended up hand wiping a few hundred plates - there were no sinks available, except in the bathrooms - and serving the desserts on those.
Normally, I would been put off a little more than I was but it was a Fashion Week party; it's not as if anyone actually ate anything.

I knew about the steak bit, and suspected the cream and chip scams. The one that surprises me is serving real coffee instead of decaf to the elderly. For someone with a heart condition, this could cause the meal to be their last one. So much for repeat business, and if the relatives are the sort for whom “Sue” is always a verb and never a given name, this could prove to be a false economy.

I worked in hotel kitchens as a kid, employed by 40s era chefs on both sides of wwII; skinflint yanks and nazis. Nothing was ever wasted. The soup pot got it and so did the help. I haven't eaten better pig slop since.
